To describe a virtual primary care (VPC) model designed to enhance primary care accessibility and efficiency.
To evaluate the operational outcomes and financial sustainability of the implemented VPC model.
To demonstrate the potential of digital tools in augmenting traditional primary care teams.

Implementation of a virtual primary care service line utilizing on-demand and asynchronous digital communication tools.
Integration of telemedicine and eVisit platforms to supplement traditional in-person primary care.
