The gallbladder is a small, pear-shaped organ that plays a crucial role in our digestive system. Measuring about 10 cm in length, it is comparable in size to a kiwi fruit and is located in a hollow area on the lower surface of the liver. Summary

Gallbladder actinomycosis, a rare infection by Actinomyces bacteria, can mimic other biliary diseases. Complete surgical removal may suffice for localized gallbladder actinomycosis, potentially avoiding prolonged antibiotic treatment.

Background:

  • Actinomycosis is a chronic infection caused by Actinomyces species, anaerobic, gram-positive bacteria.
  • These bacteria are normal flora in the oral cavity, gastrointestinal tract, and female genital tract.
  • While cervicofacial actinomycosis is common, rare presentations like gallbladder involvement can occur.

Observation:

  • A 61-year-old male presented with right upper quadrant pain, jaundice, nausea, and vomiting.
  • Imaging indicated biliary obstruction attributed to common bile duct stones.
  • The patient underwent endoscopic retrograde cholangiopancreatography and laparoscopic cholecystectomy.

Findings:

  • Histopathological examination of the gallbladder revealed Actinomyces species.
  • Identification was confirmed using Gram, PAS, and Grocott staining techniques.
  • The case highlights a rare instance of gallbladder actinomycosis.

Implications:

  • Gallbladder actinomycosis presents a diagnostic challenge, often mimicking other hepatobiliary conditions.
  • Complete surgical excision of localized infection may be a sufficient treatment.
  • This case suggests that prolonged antibiotic therapy might not always be necessary for localized gallbladder actinomycosis post-surgery.
