Summary

T1 MP2RAGE mapping reveals spinal cord changes in degenerative cervical myelopathy (DCM) patients, correlating with clinical outcomes. This quantitative MRI technique shows promise as a biomarker for surgical management of DCM.

Background:

  • Degenerative cervical myelopathy (DCM) is a leading cause of spinal cord (SC) dysfunction.
  • Conventional MRI, particularly T2-weighted imaging, depicts SC changes but often lacks clinico-radiological correlation.
  • T1 MP2RAGE sequences offer potential for quantitative SC assessment.

Purpose of the Study:

  • To evaluate the utility of quantitative T1 MP2RAGE MRI in assessing spinal cord alterations in DCM.
  • To explore the correlation between T1 MP2RAGE-derived metrics and clinical outcomes in DCM patients.

Main Methods:

  • 18 DCM patients and 17 healthy controls (HC) underwent T1 MP2RAGE and T2-weighted imaging.
  • Clinical status was assessed using the modified Japanese Orthopaedic Association (mJOA) scale.
  • Data were analyzed using Matlab and the SC Toolbox.

Main Results:

  • DCM patients showed significantly different mean SC T1 values compared to HC (p < 0.0001).
  • T1 values correlated with both preoperative and postoperative mJOA scores (R²=0.33, p=0.013 and R²=0.29, p=0.024, respectively).
  • Baseline T1 value at C2 level correlated with 3-month follow-up mJOA (p=0.048).

Conclusions:

  • T1 MP2RAGE mapping effectively detects focal and diffuse spinal cord alterations in DCM.
  • This quantitative MRI technique shows potential as a biomarker for surgically managed DCM patients.
