Local Anesthetics: Clinical Application as Intravenous Regional Anesthesia01:16

Local Anesthetics: Clinical Application as Intravenous Regional Anesthesia

Intravenous regional anesthesia or the Bier block technique is used to anesthetize a specific limb or extremity. It uses exsanguinated or blood-drained vessels to transport local anesthetics or LAs to the peripheral nerve trunks. Lidocaine without vasoconstrictors like epinephrine is most commonly used for this technique. Other drugs used are prilocaine, ropivacaine, and chloroprocaine. Bupivacaine is not recommended for this technique due to its high cardiac toxicity.

Area of Science:

  • Surgical techniques
  • Wound closure innovations
  • Thoracic surgery

Background:

  • Chest tube insertion and removal are common procedures.
  • Subcuticular purse-string closure is often used.
  • Existing methods can be time-consuming or complex.

Purpose of the Study:

  • To present a novel technique for subcuticular purse-string suture closure.
  • To utilize barbed sutures for enhanced efficiency.
  • To describe application during chest tube insertion and removal.

Main Methods:

  • A barbed suture was employed for subcuticular purse-string closure.
  • The technique was applied during chest tube insertion.
  • The technique was applied during chest tube removal.

Main Results:

  • The barbed suture technique facilitates efficient purse-string closure.
  • This method simplifies the closure process for chest tube sites.
  • The technique is applicable to both insertion and removal phases.

Conclusions:

  • Barbed sutures offer a novel and effective approach for subcuticular purse-string closure.
  • This technique streamlines chest tube management.
  • Further studies may explore broader applications of this method.
