Background:

  • Malignant pleural mesothelioma presents challenges in assessing tumor progression via imaging.
  • Objective indices independent of imaging are needed for evaluating tumor burden and surgical candidacy.

Purpose of the Study:

  • To identify objective prognostic factors for malignant pleural mesothelioma.
  • To evaluate the utility of clinical factors in predicting postoperative survival.

Main Methods:

  • Retrospective analysis of 79 patients with malignant pleural mesothelioma undergoing surgery.
  • Evaluation of clinical factors including percent predicted forced vital capacity (FVC) and imaging findings.

Main Results:

  • Low percent predicted FVC (<80%) was the sole significant independent predictor of poor overall survival (HR 2.76, P=.01).
  • Two-year survival was significantly lower for patients with FVC <80% (16.6%) compared to those with FVC ≥80% (76.6%).
  • Imaging findings and pathologic stage showed less significant prognostic value in multivariate analysis.

Conclusions:

  • Percent predicted FVC is a robust, imaging-independent predictor of postoperative survival in malignant pleural mesothelioma.
  • FVC assessment aids in evaluating tumor invasion and guiding surgical indications.
