    Summary

Histologic evidence of myocarditis is increasingly found in patients with dilated cardiomyopathy. This study found myocarditis in 30% of patients suspected of having dilated cardiomyopathy, suggesting a link between the two conditions.

    Area of Science:

    • Cardiology
    • Pathology

    Context:

    • Dilated cardiomyopathy (DCM) diagnosis often involves ruling out other conditions.
    • Endomyocardial biopsy is increasingly used to identify myocarditis in DCM patients.

    Purpose:

    • To analyze histologic, clinical, hemodynamic, and immunologic findings in patients with impaired left ventricular function.
    • To determine the relationship between histologically-documented myocarditis and dilated cardiomyopathy.

    Summary:

    • Histologic evidence of myocarditis was found in 41 of 150 patients (27%) with unexplained impaired left ventricular function.
    • In patients with active myocarditis at initial biopsy, 7 of 8 showed findings consistent with DCM after two years.
    • Pooled data from multiple studies indicate an overall incidence of 30% for histologically-documented myocarditis in patients suspected of DCM.

    Impact:

    • Findings suggest myocarditis is a significant, often undiagnosed, contributor to dilated cardiomyopathy.
    • Highlights the importance of endomyocardial biopsy in diagnosing DCM etiology.
    • Understanding this link can improve diagnostic accuracy and treatment strategies for heart failure.
