Kidney Structure

68.7K
The kidneys are two large bean-shaped organs located in the upper abdomen. They filter the blood several times a day to remove toxins and rebalance water and electrolytes of the circulatory system via the renal veins. The kidneys receive blood directly from the heart via the renal arteries. These arteries enter the kidney at the hilum, the concave surface of the bean, where they branch and divide into smaller vessels and capillaries.

Heart Failure Drugs: Diuretics

338
Heart failure and kidney perfusion are interconnected in a complex way. Reduced renal perfusion and venous congestion are two significant factors that contribute to renal dysfunction in heart failure. The kidneys, primarily responsible for fluid balance in the body, are adversely affected due to compromised cardiac output and increased venous pressure. In response to reduced renal perfusion, the kidneys activate neurohumoral mechanisms to restore balance. However, these mechanisms can be...

Area of Science:

  • Nephrology
  • Cardiology
  • Immunology

Background:

  • Cardiac surgery-associated acute kidney injury (CSA-AKI) affects approximately 25% of patients undergoing cardiac procedures, especially those requiring cardiopulmonary bypass.
  • CSA-AKI significantly elevates risks of severe morbidities and mortality, imposing a substantial burden on patients and healthcare systems.
  • Preexisting kidney dysfunction, such as chronic kidney disease, heightens the risk and worsens outcomes for patients developing CSA-AKI.

Purpose of the Study:

  • To review the current understanding of the pathophysiology and clinical management of CSA-AKI.
  • To discuss emerging therapeutic targets and future strategies for CSA-AKI prevention and treatment.

Main Methods:

  • Literature review of current research on CSA-AKI.
  • Analysis of the mechanisms involved in CSA-AKI pathogenesis.
  • Discussion of existing and potential management strategies.

Main Results:

  • CSA-AKI pathogenesis involves a complex interplay of inflammation, cell death, complement activation, immune responses, cardiopulmonary bypass, and ischemia-reperfusion injury.
  • No universally effective pharmacological therapies currently exist for CSA-AKI prevention or treatment.
  • Promising preclinical data suggest potential future therapeutic avenues.

Conclusions:

  • Further research into the complex pathophysiology of CSA-AKI is crucial for developing effective prevention and treatment strategies.
  • Identifying and targeting key pathways in CSA-AKI development holds promise for improved patient outcomes.
  • Future therapeutic interventions aim to mitigate the significant impact of CSA-AKI on cardiac surgery patients.
