Tonsillitis II: Management01:26

Tonsillitis II: Management

103
This lesson will focus on the different treatment options for managing tonsillitis, which typically depend on the cause and severity.

Tonsillitis I: Introduction01:30

Tonsillitis I: Introduction

205
Tonsillitis is inflammation of the tonsils, which are two lymphoid tissue masses at the back of the throat. This condition can cause discomfort and irritation in the throat.
Etiology
Three primary contributing factors have been identified.
205
Chronic Pharyngitis01:23

Chronic Pharyngitis

1.1K
Chronic pharyngitis refers to persistent inflammation of the pharyngial mucosa.
Etiology
It often arises from persistent viral or bacterial infections affecting sinuses and tonsils.
Additional contributing factors include inadequate dental hygiene, mouth breathing, recurring tonsillitis, allergic rhinitis, laryngopharyngeal reflux, and exposure to smoke, chemicals, and other environmental pollutants. Allergic reactions to pollen, mold, and pet dander, chronic cough, excessive voice usage,...

Epistaxis01:30

Epistaxis

123
Epistaxis, or nosebleeds, occurs when small, swollen blood vessels in the nasal mucous membrane rupture. Typically, the anterior septum is the primary site of occurrence.
Etiology
Possible causes of this condition include high blood pressure, trauma, low humidity, upper respiratory tract infections, allergies, foreign bodies, nasal inhalation of corticosteroids or illicit drugs, excessive use of decongestant nasal sprays, facial or nasal surgery, anatomic malformation, tumors, or systemic...

Pre-Procedural Guidelines for Assessing Blood Pressure01:10

Pre-Procedural Guidelines for Assessing Blood Pressure

520
Accurate blood pressure assessment is crucial for diagnosing and managing various health conditions. To ensure the reliability of these measurements, healthcare professionals must adhere to standardized pre-procedural guidelines. These guidelines enhance patient safety and improve the overall quality of healthcare. The following steps are essential for obtaining accurate and consistent blood pressure readings, from using the appropriate tools to ensuring effective communication with the...

Disorders of Hemostasis01:24

Disorders of Hemostasis

694
Hemostasis, the process that stops bleeding after a blood vessel injury, is crucial for maintaining the integrity of the circulatory system. However, disorders of hemostasis can disrupt this delicate balance, leading to either excessive clotting or bleeding. These disorders can be broadly classified into thromboembolic disorders and bleeding disorders.
Thromboembolic Disorders
Two factors primarily cause thromboembolic conditions.

Summary

Adults and patients using non-steroidal anti-inflammatory drugs (NSAIDs) post-tonsillectomy face a higher risk of post-tonsillectomy hemorrhage (PTH). Minor bleeding episodes also increase the likelihood of rebleeding in adults.

Area of Science:

  • Otolaryngology
  • Surgical Outcomes
  • Hemorrhage Research

Background:

  • Post-tonsillectomy hemorrhage (PTH) is a significant complication.
  • Understanding risk factors is crucial for patient management and preventing rebleeding.

Purpose of the Study:

  • To identify clinical factors associated with post-tonsillectomy hemorrhage (PTH).
  • To investigate the risk of rebleeding episodes after tonsillectomy.

Main Methods:

  • Retrospective review of 1,082 patient records from May 2018 to April 2019.
  • Data collected included demographics, surgical details, postoperative care, and hemorrhage events.
  • Statistical analysis using univariate and multivariable models to identify risk factors.

Main Results:

  • Overall PTH rate was 8.0%, with higher incidence in adults (18.8%) compared to children (4.0%).
  • Significant risk factors for PTH included older age (>15 years) and postoperative NSAID use.
  • Severe hemorrhage requiring surgical intervention was more common in adults (8.2%) than children (1.5%).

Conclusions:

  • Adult patients and those using NSAIDs postoperatively are at increased risk of PTH.
  • Previous minor bleeding episodes may elevate the risk of subsequent hemorrhage in adults.
