Background:

Elephantiasis nostras is a rare condition characterized by persistent swelling, typically of the lower extremities.
It is commonly secondary to recurrent episodes of lymphangitis, an inflammation of lymphatic vessels.

Observation:

A case is presented of a patient initially referred for angioneurotic edema of the lip.
The patient exhibited several months of persistent upper lip swelling.
Probable evidence of a recent lip infection was noted, consistent with elephantiasis nostras.

Findings:

The case highlights that elephantiasis nostras can manifest with chronic lip swelling, mimicking other forms of angioedema.
Facial presentation of this condition is uncommon but clinically significant.
