High alert medications off the radar: A systematic review

Certain medications not on official High Alert Medications (HAM) lists, such as risperidone and piperacillin-tazobactam, pose significant risks to critically ill patients due to medication errors. These drugs warrant consideration for inclusion in future HAM lists to improve patient safety.
