  • Utilized data from the Early Chronic Obstructive Pulmonary Disease study, including new participants and third-year follow-ups.
  • Assessed diffusion capacity (DLCO <80% predicted), spirometry-defined SAD (<65% predicted), impulse oscillometry (IOS)-defined SAD (>0.07 kPa·L-1·s difference in resistance), and CT-defined SAD (≥15% low-attenuation areas).
  • Employed covariate analyses and logistic regression to determine associations between impaired DLCO and SAD.

Main Results:

  • A significant association was found between small airway dysfunction (SAD) and impaired diffusion capacity of the lungs for carbon monoxide (DLCO).
  • The co-occurrence of spirometry- and CT-defined SAD was notably higher in individuals with impaired DLCO compared to those with normal DLCO.
