Polyneuropathy due to polyangiitis overlap syndrome.

L H Kortbeek, M W van Beek

Journal of Neurology, Neurosurgery, and Psychiatry
    |February 1, 1985
    PubMed
    Summary

A patient with mild polyneuropathy experienced a fatal intra-abdominal hemorrhage and cardiac arrest. Autopsy revealed polyangiitis overlap syndrome affecting multiple organs, with myocarditis identified as the cause of death.

    Area of Science:

    • Medicine
    • Pathology
    • Rheumatology

    Background:

    • Polyneuropathy can be associated with systemic inflammatory conditions.
    • Rarely, vasculitis may present with severe intra-abdominal complications.

    Observation:

    • A patient with pre-existing mild polyneuropathy developed sudden, severe intra-abdominal hemorrhage.
    • The patient rapidly deteriorated, succumbing to cardiac arrest within days.

    Findings:

    • Microscopic examination confirmed polyangiitis overlap syndrome (Fauci), a systemic vasculitis.
    • Lesions were identified in multiple organs, including peripheral nerves and the heart.
    • Myocarditis was determined to be the direct cause of cardiac arrest.

    Implications:

    • This case highlights the potential for severe, life-threatening complications in polyangiitis overlap syndrome.
    • Early recognition and management of vasculitis are crucial, even with seemingly mild initial symptoms.
    • Systemic vasculitis can manifest with diverse and critical organ involvement, including cardiovascular complications.
