    Transpubic urethroplasty for traumatic prostatomembranous strictures can lead to erectile dysfunction and urinary incontinence. Prosthetic devices may help manage these challenging sequelae in patients.

    Background:

    • Traumatic prostatomembranous strictures are complex injuries.
    • Transpubic urethroplasty is a surgical option for these strictures.

    Purpose of the Study:

    • To evaluate the long-term outcomes of transpubic urethroplasty.
    • To identify the sequelae of this surgical procedure.

    Main Methods:

    • Retrospective review of 44 patients undergoing transpubic urethroplasty from 1973-1982.
    • Follow-up duration of 1 to 10 years.
    • Assessment of late sequelae including erectile dysfunction and urinary incontinence.

    Main Results:

    • 22 patients (50%) experienced permanent erectile dysfunction.
    • 4 patients (9%) had progressive erectile dysfunction.
    • 14% had significant primary urinary incontinence requiring a genitourinary sphincter.
    • 4.5% developed incontinence after secondary procedures.
    • 10 of 13 patients (77%) with post-urethroplasty strictures required dilation.

    Conclusions:

    • Transpubic urethroplasty for traumatic prostatomembranous strictures is associated with high rates of erectile dysfunction and urinary incontinence.
    • Prosthetic devices offer a potential solution for managing these complications.
    • Further management may be needed for post-urethroplasty strictures.
