Background:

  • Internal root resorption (IRR) presents challenges in management, especially with extensive apical lesions.
  • Traditional root canal treatment (RCT) and obturation techniques have limitations for apical IRR cases.

Observation:

  • A 49-year-old male patient presented with bilateral apical IRR and large endodontic lesions in mandibular first molars.
  • Despite previous RCT, persistent symptoms and radiographic evidence of apical lesions and resorptive defects were noted.

Findings:

  • Nonsurgical retreatment with bio-obturation using a calcium-enriched mixture cement was performed.
  • Complete healing of apical lesions was observed radiographically at five years post-treatment.
  • Patient symptoms resolved within one week of the bio-obturation procedure.
