    Background:

    • Pulmonary hypertension (PH) is a serious complication of chronic obstructive pulmonary disease (COPD).
    • Accurate assessment of PH in COPD patients is crucial for management.

    Purpose of the Study:

    • To evaluate the correlation between pulmonary function tests (PFTs), capillary blood gas (CBG) values, and pulmonary hemodynamic parameters in COPD patients with PH.
    • To identify the most effective PFTs for diagnosing PH in this population.

    Main Methods:

    • Seventy-four COPD patients with PH were assessed.
    • Pulmonary function tests, capillary blood gas analysis, and pulmonary hemodynamic measurements were performed.
    • Patients were categorized based on the presence of PH at rest or during exercise.

    • A significant correlation was found between low capillary oxygen tension and elevated mean pulmonary artery pressure (mPAP) and pulmonary vascular resistance (PVR) at rest and during exercise.

    • Reduced FEV1/VC ratio and low resting or exercise-induced oxygen tension are the most effective pulmonary function test criteria for detecting pulmonary hypertension in COPD patients.
    • Capillary oxygen tension is a more significant predictor of PH severity than capillary carbon dioxide tension in this cohort.
