    Background:

    • Prolonged septic shock is associated with progressive myocardial dysfunction.
    • Waterhouse-Friderichsen syndrome, a severe complication of meningococcal infections, presents unique cardiovascular challenges.

    Observation:

    • Two patients with meningococcal infection and Waterhouse-Friderichsen syndrome exhibited early hyperdynamic septic shock.
    • Rapidly progressive cardiac failure with low cardiac output and high filling pressures developed subsequently.

    Findings:

    • Acute myocarditis is a frequent finding in meningococcal infections and is suggested as the cause of acute myocardial failure.
    • One patient experienced irreversible shock, while the other showed improvement with inotropic agents and gradual recovery of left ventricular function.

    Implications:

    • Early recognition and management of cardiac dysfunction in septic shock are crucial.
    • Inotropic support may be beneficial in mitigating myocardial damage and improving outcomes in severe sepsis-induced cardiomyopathy.
    • Further research into the mechanisms of sepsis-induced myocarditis is warranted.
