    Area of Science:

    • Gastroenterology
    • Obstetrics
    • Surgical Case Report

    Background:

    • Choledochal cysts (bile duct dilatation) pose significant risks during pregnancy.
    • Pregnancy with a choledochal cyst requires careful management due to potential complications.

    Observation:

    • A rare case of choledochal cyst occurred in a twin gestation.
    • The overdistended uterus presented an urgent surgical challenge.

    Findings:

    • A conservative surgical approach was successfully employed during the antepartum period.
    • This intervention led to a successful outcome for both mother and fetuses.

    Implications:

    • Highlights the feasibility and safety of conservative surgical management for choledochal cysts in pregnancy.
    • Provides a valuable case study for obstetricians and gastroenterologists managing similar high-risk pregnancies.
