Tumor progression is a phenomenon where the pre-formed tumor acquires successive mutations to become clinically more aggressive and malignant. In the 1950s, Foulds first described the stepwise progression of cancer cells through successive stages.

  • Understanding IC growth patterns is crucial for optimizing screening strategies.

Purpose of the Study:

  • To estimate the tumour volume doubling time (TVDT) of interval cancers (ICs).
  • To correlate TVDT with clinicopathological variables.
  • To estimate the size of non-visible ICs at screening.

Main Methods:

  • Retrospective review of mammograms by two radiologists to measure IC diameter.
  • Univariate and generalized linear model analyses of clinicopathological variables (ER, HER2, grade, age, breast density).
  • Estimation of TVDT, screening size, and visibility time for non-visible tumours.

Main Results:

  • Median TVDT varied significantly by cancer grade (Grade 1: 317 days, Grade 2: 288 days, Grade 3: 195 days).
  • TVDT was significantly associated with age and ER status.
  • Estimated median size of non-visible cancers at screening ranged from 0.9 mm (Grade 3) to 2.5 mm (Grade 2).

Conclusions:

  • TVDT of visible ICs can estimate the size of non-visible ICs at screening.
  • Increasing screening frequency from three-yearly to two-yearly could significantly reduce ICs.
