Background:

  • Family history (FH) of psoriasis is a known risk factor for disease development.
  • The impact of FH on psoriasis clinical presentation and treatment response to biologics is not well understood.

Purpose of the Study:

  • To investigate differences in clinical presentation and biologic treatment efficacy in psoriasis patients with and without a family history of the disease.

Main Methods:

  • Prospective, multicenter observational study using the SPEECH registry.
  • Included adult patients with moderate-to-severe plaque psoriasis initiating biologic therapy.
  • Assessed family history via self-report (first-degree relative). Primary outcomes: PASI75 and PGA 0/1 at 3 months. Logistic regression used for analysis.

Main Results:

  • 859 patients included; 22.9% had a positive FH.
  • Patients with FH had earlier onset and more severe anxiety.
  • Higher likelihood of achieving PASI75 (aOR 1.60) and PGA 0/1 (aOR 1.54) at 3 months in FH+ group.
  • Differences persisted at 6 months; age of onset mediated 20.5% of the effect.

Conclusions:

  • Family history of psoriasis may influence clinical course and improve response to biologics.
  • FH assessment is important for optimizing treatment outcomes and guiding biologic selection.
  • Family history should be considered a significant factor in future psoriasis biologic treatment studies.
