Background:

  • Parathyroid incidentalomas (PTIs) detected via ultrasonography (US) are often misdiagnosed as perithyroidal lymph nodes due to reliance on clinical experience.
  • Accurate differentiation of PTIs is essential for appropriate patient management.

Purpose of the Study:

  • To evaluate the diagnostic role of US in detecting PTIs.
  • To define the clinicopathologic features of PTIs identified during routine neck US examinations.

Main Methods:

  • Retrospective analysis of 41,275 neck US reports.
  • Comparison of patients with pathology-confirmed PTIs (Group PCD) versus those without (Group NPCD).
  • Statistical analysis of demographic data, laboratory results (PTH, calcium, anti-TPO, anti-TG), and imaging findings.

Main Results:

  • Parathyroid incidentalomas (PTIs) were suspected in 66 patients (0.16%).
  • Pathological confirmation of PTI was obtained in 31 operated patients.
  • Significantly higher PTH and calcium levels were observed in Group PCD, while higher anti-TPO and anti-TG levels were noted in Group NPCD.

Conclusions:

  • Ultrasonography (US) findings suggestive of parathyroid incidentalomas (PTIs) warrant further investigation with laboratory tests and advanced imaging.
  • Establishing a multidisciplinary approach involving nuclear medicine, general surgery, and pathology is vital for accurate diagnosis and management of PTIs.
