    Summary

    Argon laser trabeculoplasty effectively lowers intraocular pressure in patients with uncontrolled primary open-angle glaucoma. The greatest pressure reduction often occurred after the first treatment stage.

    Background:

    • Primary open-angle glaucoma (POAG) is a leading cause of irreversible blindness.
    • Uncontrolled POAG despite maximal medical therapy necessitates alternative treatment strategies.
    • Progressive optic disc cupping and visual field loss indicate disease progression.

    Purpose of the Study:

    • To evaluate the efficacy of two-stage argon laser trabeculoplasty (ALT) in patients with medically uncontrolled POAG.
    • To assess the impact of pretreatment intraocular pressure (IOP) on treatment response.
    • To document the safety and complications associated with two-stage ALT.

    Main Methods:

    • Forty-five adult patients with POAG underwent two-stage ALT (180 degrees per session, one month apart).
    • Indications included progressive optic disc cupping and visual field loss despite maximal medical therapy.
    • Intraocular pressure (IOP) was analyzed using Student's two-tailed paired t-test (P < .01 for significance).

    Main Results:

    • The greatest IOP reduction was typically observed after the first ALT stage.
    • Response to laser therapy was influenced by baseline IOP; higher pretreatment pressures yielded greater benefits.
    • A transient postoperative IOP increase (≥5 mm Hg) occurred in approximately 19% of patients.

    Conclusions:

    • Two-stage ALT is an effective treatment for lowering IOP in patients with uncontrolled POAG.
    • The degree of IOP reduction is related to the initial IOP level.
    • While generally safe, transient IOP spikes are a potential complication of ALT.
