Background:

  • Attention Deficit/Hyperactivity Disorder (ADHD) is a neurodevelopmental disorder characterized by inattention and/or hyperactivity-impulsivity.
  • Emotional processing difficulties are frequently anecdotally reported in individuals with ADHD, but empirical evidence remains fragmented.
  • A systematic review is needed to consolidate existing research and quantify the extent of emotional processing deficits in ADHD.

Purpose of the Study:

  • To systematically review and meta-analyze empirical studies on emotional processing in individuals diagnosed with ADHD compared to non-psychiatric controls.
  • To determine if emotional processing deficits in ADHD are global or specific to certain emotional categories.
  • To identify potential moderators, such as age, sex, medication status, stimulus type, and outcome measurement, influencing emotional processing in ADHD.

Main Methods:

  • A PRISMA-compliant systematic review and Bayesian meta-analysis were conducted.
  • Searched major databases (MEDLINE, PsycINFO, ERIC, Scopus, Web of Science) until December 3, 2023.
  • Included 80 studies (6191 participants) comparing individuals with ADHD (DSM or ICD criteria) to controls, assessing study quality with AXIS.

Main Results:

  • Individuals with ADHD showed significantly poorer overall emotional processing compared to controls (g = -0.65).
  • Deficits were observed across all specific emotional categories, including anger, disgust, fear, sadness, surprise, happiness/positive, negative, and neutral emotions.
  • Scale items and accuracy as outcome measures were the most effective moderators; age, sex, and medication status did not significantly moderate the effects.

Conclusions:

  • Impaired emotional processing is a pervasive and relevant feature of Attention Deficit/Hyperactivity Disorder.
  • These deficits extend across various emotional categories, suggesting a global impairment rather than category-specific issues.
  • Systematic assessment of emotional processing should be integrated into the clinical evaluation and management of individuals with ADHD.
