    Background:

    • Cerebellar venous angiomas (CVAs) are rare vascular malformations.
    • Understanding their natural history and optimal treatment is crucial.

    Observation:

    • Three patients with CVA were studied: one with cerebellar hematoma undergoing surgery with a fatal outcome, and two with unruptured CVAs managed conservatively with stable or improved symptoms.
    • A review of 24 additional cases did not reveal definitive trends.

    Findings:

    • Surgical excision of CVA associated with cerebellar hematoma carried a high risk of mortality.
    • Conservative management of unruptured CVA resulted in stable or asymptomatic outcomes in the observed cases.

    Implications:

    • Conservative treatment is a logical approach for patients with unruptured cerebellar venous angiomas.
    • Further research is needed to establish definitive treatment guidelines for CVA.
