    Choroidal hyperpermeability in central serous choroidopathy: a new concept?

    Background:

    • Ab interno resection of retina and choroid adjacent to the optic disc is a complex surgical procedure.
    • Maintaining intraocular pressure and structural integrity post-resection is critical for visual outcomes.

    Purpose of the Study:

    • To evaluate the efficacy of sodium hyaluronate (Healon) as a vitreous replacement fluid after extensive retinal and choroidal resection in primates.
    • To assess the impact of different vitreous replacement fluids on preventing post-operative complications such as phthisis bulbi.

    Main Methods:

    • Primates underwent ab interno resection of a 6x7-disc diameter area of retina and choroid near the optic disc.
    • The resection area was pre-treated with xenon arc photocoagulation.
    • Surgical removal of tissue was performed using a wide-angle cutter vitrophage.
    • Vitreous cavity was replaced with either sodium hyaluronate (Healon) or saline solution in separate groups.

    Main Results:

    • Eyes where vitreous fluid was replaced with saline solution predominantly became phthisical (developed shrinkage and loss of function).
    • Eyes where vitreous fluid was replaced with sodium hyaluronate (Healon) largely recovered preoperative intraocular pressures.
    • The integrity of intraocular structures was maintained in almost all eyes treated with sodium hyaluronate.

    Conclusions:

    • Sodium hyaluronate (Healon) is an effective vitreous substitute for maintaining intraocular pressure and structural integrity after large retinal and choroidal resections.
    • The use of sodium hyaluronate significantly reduces the incidence of phthisis bulbi compared to saline solution in this surgical model.
    • This study highlights the potential of Healon in improving outcomes for complex retinal surgeries requiring significant tissue removal.
