Background:

  • Endolymphatic sac tumors (ELSTs) are rare neoplasms originating in the endolymphatic capsule.
  • Diagnosis is challenging due to low prevalence, slow progression, and nonspecific symptoms.
  • Prompt surgical intervention is the recommended treatment upon diagnosis.

Observation:

  • A 53-year-old male presented with dizziness and blurred vision, later diagnosed with an endolymphatic sac tumor.
  • The tumor, located in the left internal auditory canal, temporal bone mastoid, and jugular foramen, exhibited aggressive growth and rich vascularity.
  • Surgical resection was complex due to the tumor's proximity to vital neurovascular structures.

Findings:

  • Postoperative pathology confirmed the lesion as an endolymphatic cystic tumor.
  • Complete surgical resection was achieved while preserving neurological function.
  • Adjuvant gamma knife treatment was administered for residual tumor with positive outcomes.

Implications:

  • Complete surgical resection is the primary treatment for ELSTs.
  • Adjuvant therapies like radiotherapy or gamma knife may be necessary for residual disease.
  • Preoperative embolization could aid surgical resection by reducing intraoperative bleeding.
