    Summary

Behcet's syndrome can cause elevated liver enzymes and weakness, potentially indicating liver involvement. Liver biopsy revealed inflammation, suggesting a shared autoimmune basis with chronic active hepatitis.

    Area of Science:

    • Hepatology
    • Immunology
    • Rheumatology

    Background:

    • Behcet's syndrome is a multisystem vasculitis with uncertain etiology.
    • Elevated serum transaminases suggest potential hepatic involvement in Behcet's syndrome.

    Observation:

    • A patient with Behcet's syndrome presented with significantly elevated serum transaminase levels (5x normal) and weakness.
    • Liver needle biopsy demonstrated piecemeal necrosis and portal lympho-monocytic infiltration.

    Findings:

    • The presence of HLA B5 and B27 antigens was noted in the patient.
    • Histopathological findings indicate chronic active hepatitis.

    Implications:

    • The simultaneous occurrence of Behcet's syndrome and chronic active hepatitis suggests a potential shared autoimmune pathogenesis.
