Background:

Crohn's disease frequently leads to intestinal strictures due to chronic inflammation and healing cycles.
Ileal strictures are a common complication, often causing small intestinal obstruction.

Observation:

A 35-year-old male with an 18-year history of Crohn's disease presented with a 3-month history of subacute small intestinal obstruction.
MR enterography revealed a 6-cm kinked ileal stricture, with proximal small bowel dilation and distal collapse observed during surgery.

Findings:

The patient underwent surgical resection of the stenosed ileal segment.
A side-to-side stapled anastomosis was performed, resulting in a smooth recovery and marked improvement.

Implications:

Early surgical intervention is recommended for symptomatic Crohn's disease strictures.
Side-to-side stapled anastomosis appears to be a favorable surgical technique for ileal stricture resection in Crohn's disease.
