    Summary

    Twice-daily prazosin dosing did not significantly alter blood pressure compared to once-daily dosing in hypertensive patients. However, reducing dosing frequency to once daily led to adverse effects like dizziness in some patients.

    Background:

    • Essential hypertension management requires optimized medication regimens.
    • Prazosin is an effective antihypertensive medication, but its optimal dosing frequency is debated.

    Purpose of the Study:

    • To compare the efficacy and tolerability of once-daily versus twice-daily prazosin administration in patients with essential hypertension.

    Main Methods:

    • A randomized cross-over study involving 20 patients with essential hypertension.
    • Blood pressure was monitored in-clinic and at-home using standardized sphygmomanometers.
    • Concurrent antihypertensive medications were kept constant throughout the study.

    Main Results:

    • No significant differences in morning or evening blood pressure were observed between once-daily and twice-daily prazosin.
    • Eight patients reported dizziness and faintness with once-daily dosing, which resolved with twice-daily administration.
    • Subjective adverse effects did not correlate with serum prazosin levels.
