    Area of Science:

    • Dermatology
    • Surgical Oncology

    Background:

    • Basal cell epithelioma (BCE) is the most common form of skin cancer.
    • Pigmented variants of BCE can sometimes be challenging to diagnose clinically.

    Observation:

    • A 73-year-old male presented with a 20-mm linear pigmented lesion on his left infraorbital cheek.
    • Initial clinical impression suggested a pigmented basal cell epithelioma.

    Findings:

    • Surgical excision under local anesthesia was performed.
    • Microscopic examination confirmed nodular basal neoplastic cells consistent with basal cell epithelioma.
    • Melanin deposits were noted within the tumor and stroma, including in melanophages.
    • Surgical margins were clear of tumor on transverse sectioning.

    Implications:

    • This case highlights the successful surgical management of a pigmented basal cell epithelioma.
    • The procedure resulted in excellent cosmetic outcomes.
    • Accurate histopathological examination is crucial for confirming diagnosis and ensuring complete tumor removal.
