A transannular and main pulmonary artery patch repair for Fallot's tetralogy (ToF) can lead to acute coronary artery compression after Harmony transcatheter pulmonary valve (TPV) deployment. Emergent surgery with bioprosthetic valve replacement was required to address this complication.
