Summary

A rare aortic dissection occurred in the recipient aorta 10 years after heart transplant surgery. Prompt surgical repair is vital for managing this life-threatening complication in heart transplant recipients.

Area of Science:

  • Cardiovascular Surgery
  • Transplant Surgery
  • Aortic Disease

Background:

  • Orthotopic heart transplantation is a life-saving procedure for end-stage heart failure.
  • Aortic dissection is a rare but critical complication following heart transplantation.
  • Donor-recipient aortic anastomosis is the typical site for dissections.

Observation:

  • A 40-year-old male presented with chest pain and dyspnea 10 years post-heart transplant.
  • Imaging revealed a chronic dissection originating from the donor-recipient aortic suture line, extending to the iliac arteries.
  • A donor-recipient aortic size mismatch was noted during the initial transplant surgery.

Findings:

  • The dissection occurred in the recipient aorta, unlike most reported cases involving the donor aorta.
  • Surgical intervention included ascending aorta replacement with a Dacron graft and anastomosis reinforcement.
  • Postoperative follow-up showed complete thrombosis of the false lumen without complications.

Implications:

  • Aortic dissection post-heart transplant necessitates immediate surgical intervention due to high mortality.
  • Meticulous surgical technique, addressing aortic size mismatch, is crucial during initial transplantation.
  • Vigilant long-term patient monitoring is essential for preventing and managing post-transplant aortic complications.
