Oophoropexy to round ligament for recurrent ovarian torsion

Abstract:
Ovarian torsion is a gynaecological emergency commonly affecting women of reproductive age, often associated with ovarian or paraovarian masses. However, in the absence of these masses, torsion can still occur, potentially indicating an underlying anatomical abnormality that predisposes to recurrent torsion. This report presents a distinctive surgical technique used in a young girl in her early adolescence with recurrent right ovarian torsion. The patient, with a history of torsion managed by detorsion, presented again with symptoms suggestive of recurrence. Emergency diagnostic laparoscopy revealed a twisted but viable right ovary at risk of future torsion. To prevent a recurrence, a new approach was employed: the right ovarian ligament was plicated with 2-0 prolene, and the ovary was fixed to the right round ligament. The procedure successfully stabilised the ovary and prevented further torsion. Postoperative recovery was uneventful. This case demonstrates the potential of oophoropexy as an effective technique for managing recurrent ovarian torsion in adolescents.
