Bickerstaff brainstem encephalitis preceded central nervous system (CNS) relapse of diffuse large B-cell lymphoma (DLBCL). Prompt diagnosis and plasma exchange improved neurological symptoms, but CNS relapse eventually occurred, highlighting the need for careful differential diagnosis.
