Summary

Adjuvant chemotherapy may not benefit high-risk stage II colon cancer patients. This study suggests it does not improve overall survival when considering early mortality, questioning its routine use.

Background:

  • Adjuvant chemotherapy is recommended for high-risk stage II colon cancer.
  • The benefit of adjuvant therapy in all high-risk stage II colon cancer patients remains uncertain.

Purpose of the Study:

  • To evaluate the association between adjuvant chemotherapy and overall survival in high-risk stage II colon cancer patients.

Main Methods:

  • Analysis of the Surveillance, Epidemiology and End Results (SEER) database (2010-2019).
  • Inclusion of adult patients with high-risk stage II colon cancer (T4, perineural invasion, <12 lymph nodes, poor differentiation).
  • 1:1 propensity score matching, log-rank test, and Cox proportional hazards regression were used.

Main Results:

  • Of 11,619 patients, 24% received adjuvant chemotherapy.
  • Initial analysis showed improved overall survival with chemotherapy (HR 0.70; p<0.001).
  • Landmark analysis excluding patients who died within 3 months found no significant survival benefit (HR 0.90; p=0.144).

Conclusions:

  • Findings suggest that adjuvant chemotherapy may not be beneficial for patients with high-risk stage II colon cancer.
  • The study supports reconsidering routine adjuvant chemotherapy in this patient group.
