Summary

Pulmonary vein isolation (PVI) and posterior wall isolation (PWI) using cryoballoon ablation did not significantly impact left atrial mechanical function. Cardiac MRI showed no changes in left atrial ejection fraction after either procedure alone or in combination.

Background:

  • Pulmonary vein isolation (PVI) is a standard treatment for atrial fibrillation (AF).
  • Posterior wall isolation (PWI) is often used with PVI, but its effect on left atrial function is unclear.
  • Previous studies suggest PVI alone does not impair left atrial function.

Purpose of the Study:

  • To evaluate the impact of cryoballoon PVI with or without PWI on left atrial mechanical function.
  • To assess changes in left atrial function using cardiac magnetic resonance (CMR) imaging.

Main Methods:

  • A study of 28 AF patients undergoing cryoballoon ablation.
  • Patients were divided into two groups: PVI alone (14 patients) and PVI plus PWI (14 patients).
  • Cardiac MRI was performed before and after ablation to measure left atrial ejection fraction (LAEF) and volumes.

Main Results:

  • No significant differences in patient demographics or AF characteristics were observed between groups.
  • No statistically significant changes in LAEF, LA max volume, LA min volume, or LA stroke volume were found from baseline to follow-up in either the PVI-only or PVI plus PWI groups.
  • Linear regression analysis confirmed no significant differences in changes of these parameters between the two groups.

Conclusions:

  • Cryoballoon PVI alone does not significantly alter left atrial mechanical function.
  • Cryoballoon PVI combined with PWI also does not significantly impact left atrial mechanical function.
  • Cardiac MRI is a valuable tool for assessing atrial function after ablation procedures.
