[Prognostic Impact of Onodera's Prognostic Nutritional Index in Patients with Pathological Stage Ⅲ Colorectal Cancer

Objective:
In this study, we sought to evaluate the prognostic impact of Onodera's prognostic nutritional index(OPNI)in patients with pathological stage(pStage)Ⅲ colorectal cancer(CRC)following radical resection.
Method:
A total of 301 patients with pStage Ⅲ CRC who underwent radical resection at our hospital between September 2007 and August 2023 were included in this study. The patients were categorized into 2 groups according to OPNI: those with OPNI of <40 as a low OPNI group and those with OPNI of ≥40 as a high OPNI group. Multivariate analyses were performed using the Cox proportional hazards test to identify factors associated with overall survival(OS)and relapse-free survival(RFS), respectively. Survival curves of the patients for OS and RFS according to the presence or absence of adjuvant chemotherapy(ACT)were constructed using Kaplan-Meier method and compared using the log-rank test in the high and low OPNI group, respectively.
Result:
Multivariate analyses showed that low OPNI was a significantly independent risk factor for a poor prognosis of OS and RFS, respectively. The patients with ACT had significantly better outcome in the high OPNI group, but not significantly in the low OPNI group for OS and RFS, respectively.
Conclusion:
OPNI is helpful for predicting the prognosis and ACT decision-making in patients with pStage Ⅲ CRC following radical resection.
