Summary

Tumor viability did not predict local recurrence in high-grade soft tissue sarcoma (STS). Margin status, not viability, was the key factor for local recurrence risk in STS patients. Further research is needed.

Background:

  • Histopathologic assessment of tumor viability is a potential prognostic factor in various cancers.
  • High-grade soft tissue sarcoma (STS) outcomes may be predicted by tumor viability.
  • This study investigates the prognostic significance of viability in STS, considering adjuvant therapies and clinical factors.

Purpose of the Study:

  • To evaluate the prognostic significance of tumor viability in high-grade soft tissue sarcoma.
  • To determine if neoadjuvant treatments influence tumor viability in STS.
  • To identify factors predicting local recurrence in STS patients.

Main Methods:

  • Retrospective chart review of 147 patients with high-grade STS (2010-2021).
  • Collection of perioperative, clinical, and surveillance data.
  • Histopathologic analysis by a pathologist to determine tumor viability.

Main Results:

  • No significant differences in clinical variables between viability groups (≤10% vs >10%).
  • Neoadjuvant treatments, grade, size, and depth did not independently affect tumor viability.
  • No statistically significant decrease in local recurrence risk for ≤10% viability group (HR=1.19). Margin status was the only significant predictor of local recurrence.

Conclusions:

  • Neoadjuvant therapies (radiotherapy, chemotherapy) did not predictably influence STS tumor viability.
  • High necrosis rates in untreated tumors may confound treatment effect interpretation.
  • Further investigation into STS tumor viability and necrosis is needed, particularly in large collaborative studies.
