Summary

Repetitive transcranial magnetic stimulation (rTMS) combined with intensive speech therapy significantly improved aphasia recovery in stroke survivors. This adjuvant therapy enhanced speech production and language function more than therapy alone.

Background:

  • Poststroke aphasia recovery can be improved with intensive speech therapy.
  • Repetitive transcranial magnetic stimulation (rTMS) may augment outcomes when paired with speech therapy.

Purpose of the Study:

  • To evaluate the efficacy of 1-Hz rTMS over the right pars triangularis combined with multimodality aphasia therapy (M-MAT) in chronic poststroke aphasia.
  • To assess if rTMS enhances outcomes compared to sham stimulation when added to M-MAT.

Main Methods:

  • A double-blind, sham-controlled randomized clinical trial involving 44 participants with chronic nonfluent aphasia.
  • Participants received 35 hours of M-MAT over 10 days, combined with either active rTMS or sham stimulation.
  • The primary outcome measure was the Western Aphasia Battery aphasia quotient (WAB-AQ) assessed at 3 and 15 weeks.

Main Results:

  • Both groups showed improvement in WAB-AQ scores from baseline to 15 weeks.
  • A significant group-by-time interaction indicated greater improvement over time in the rTMS group (p=0.024).
  • At 15 weeks, the rTMS group had significantly higher WAB-AQ scores, less word-finding difficulty, and improved sentence production compared to sham (p=0.022).

Conclusions:

  • Intensive M-MAT alone improves speech production in chronic poststroke aphasia.
  • Combining 1-Hz rTMS with M-MAT leads to supplemental improvements in aphasia severity.
  • rTMS shows promise as an effective adjuvant therapy for M-MAT in poststroke aphasia rehabilitation.
