Radiological Investigation III: Pulmonary Angiogram and PET Scan

722
Radiological investigations are paramount in the diagnosis and management of various pulmonary diseases. Two essential investigations are the Pulmonary Angiogram and the Positron Emission Tomography (PET) Scan.
Pulmonary Angiogram
A Pulmonary Angiogram is an invasive procedure involving injecting a contrast medium through a catheter threaded into the pulmonary artery or the right side of the heart to visualize the pulmonary vasculature. Computed Tomography (CT) scans have mainly replaced this...

Area of Science:

  • Radiology and Imaging
  • Oncology
  • Pulmonary Medicine

Background:

  • Distinguishing lung metastasis from a second primary lung cancer is critical for optimal patient treatment.
  • Lung biopsy, while definitive, is an invasive diagnostic procedure.
  • Noninvasive methods are needed to improve diagnostic accuracy and patient management.

Purpose of the Study:

  • To evaluate the efficacy of computed tomography (CT) histogram analysis.
  • To determine if CT histogram analysis can noninvasively differentiate solitary pulmonary nodules as either lung metastasis or a second primary lung cancer.

Main Methods:

  • Retrospective analysis of CT images from patients with histopathologically confirmed solitary pulmonary nodules.
  • Extraction and statistical analysis of histogram features from the lesion and perilesional areas.
  • Assessment of diagnostic performance using sensitivity, specificity, and ROC curve analysis.

Main Results:

  • Second primary lung cancers had a significantly larger mean tumor diameter compared to lung metastases.
  • Lower mean skewness and entropy values were observed in the lesion area for second primary lung cancers.
  • Significant differences in standard deviation, skewness, kurtosis, and entropy were found in the perilesional area between the two groups.

Conclusions:

  • CT histogram analysis demonstrates potential as a noninvasive tool.
  • This method may aid in differentiating lung metastasis from second primary lung cancer in solitary pulmonary nodules.
  • Further validation could support its integration into clinical practice.
