Abstract

Insights

Trastuzumab-containing therapies show promise for improving survival in HER-2 positive gastric cancer patients undergoing resection. Routine HER-2 testing and targeted treatments are recommended for better outcomes.

Area of Science:

  • Oncology
  • Gastroenterology
  • Molecular Biology

Background:

  • Gastric cancer (GC) has a poor prognosis, especially when human epidermal growth factor receptor-2 (HER-2) is overexpressed.
  • Trastuzumab has improved outcomes in advanced GC, but its efficacy in resectable disease requires further study.

Purpose of the Study:

  • To evaluate the impact of trastuzumab-containing therapies on overall survival (OS) in patients with resectable HER-2 positive gastric cancer.
  • To identify prognostic factors for survival in gastric cancer patients.

Main Methods:

  • Retrospective analysis of 180 patients undergoing total gastrectomy with D2 lymph node dissection (2016-2021).
  • HER-2 status determined for 97 patients; 20 HER-2 positive patients received trastuzumab-containing therapies.
  • Comparison of OS between HER-2 positive (trastuzumab-treated) and HER-2 negative groups, with univariate and multivariate analyses.

Main Results:

  • HER-2 positivity was initially associated with improved OS (p=0.038).
  • Univariate analysis showed HER-2 positivity, younger age, advanced tumor stage, and larger tumor size were significant for OS.
  • Multivariate analysis identified advanced tumor stage and younger age as independent predictors of worse survival; HER-2 positivity and tumor size lost significance.

Conclusions:

  • Trastuzumab-containing treatments may significantly improve survival in resectable HER-2 positive GC.
  • Routine HER-2 assessment and targeted therapies are recommended to enhance patient outcomes.
  • Advanced stage and younger age are key prognostic factors in gastric cancer.
