This project was initiated to reduce hospital-acquired pressure injuries among vascular surgery patients by implementing the Scott Triggers tool for perioperative patients. The study was conducted at a 415-bed acute care hospital in southwest Ohio. The Scott Triggers tool, used to assess patient risk factors, was integrated into perioperative care to enhance communication and increase preventive measures. Over nine weeks, the tool was applied to 145 patients, identifying 118 as high-risk. Although the reduction in hospital-acquired pressure injuries was not statistically significant because of the small sample size, the project highlighted the tool's potential effectiveness. Key findings included the importance of accurate risk assessment, effective communication, and staff member education. Continuous education and support for staff members are essential to sustain improvements in patient outcomes and ensure high standards of care.
