Background:

  • Metastatic melanoma (mM) patients often exhibit resistance to immune checkpoint inhibitors (ICIs).
  • There is a critical need for noninvasive biomarkers to identify non-responders early in ICI treatment.
  • Early prediction of treatment response can guide therapeutic decisions and improve patient outcomes.

Purpose of the Study:

  • To evaluate the prognostic role of early [18F]2-fluoro-2-deoxy-D-glucose PET/CT ([18F]-FDG PET/CT) at four weeks (W4) in patients with metastatic melanoma receiving ICIs.
  • To assess other potential prognostic biomarkers, including serum lactate dehydrogenase (LDH) levels, metastatic site count, and immune-related adverse events (irAEs), for survival prediction.

Main Methods:

  • A prospective, noninterventional study involving metastatic melanoma patients treated with ICIs.
  • [18F]-FDG PET/CT scans were performed at baseline, W4, week 16, and every 16 weeks thereafter.
  • Tumor response at W4 was assessed using modified EORTC criteria, categorizing patients into clinical benefit (CB) and no clinical benefit (no-CB) groups. Survival analysis was conducted using Kaplan-Meier and Cox regression.

Main Results:

  • Out of 71 patients, 51 (72%) were in the CB group and 17 (24%) in the no-CB group.
  • A statistically significant difference in median overall survival (OS) was observed between groups (NR for CB vs. 6.2 months for no-CB; p=0.003).
  • Normal serum LDH levels and the occurrence of irAEs (especially cutaneous) were associated with significantly longer median OS.

Conclusions:

  • Early [18F]-FDG PET/CT at W4 serves as a valuable prognostic imaging biomarker for metastatic melanoma patients undergoing ICI therapy.
  • Serum LDH levels and the development of cutaneous irAEs are also significant prognostic indicators for survival in this patient cohort.
  • These early assessments can help stratify patients and potentially optimize treatment strategies for metastatic melanoma.
