Summary

Mental practice (MP) shows promise for improving upper extremity (UE) function in stroke survivors during acute inpatient rehabilitation (AIR). This pilot study found MP feasible and appropriate for patients with severe UE hemiparesis.

Background:

  • Severe upper extremity (UE) hemiparesis is a common challenge post-stroke.
  • Limited interventions exist for this population in acute inpatient rehabilitation (AIR).
  • The feasibility of mental practice (MP) in AIR for severe UE hemiparesis remains unexplored.

Purpose of the Study:

  • To assess the effect of MP on severe UE hemiparesis.
  • To determine the feasibility of implementing MP in the AIR setting.

Main Methods:

  • Pilot study with a single-group, pretest-posttest design.
  • Eleven patients (<1-month post-stroke, ages 18-90) with severe UE hemiparesis underwent a 2-week MP protocol (5 days/week).
  • UE function and impairment were measured using the Wolf Motor Function Test (WMFT) and Fugl Meyer Assessment (FM).
  • Feasibility was assessed via surveys of patients and occupational therapists.

Main Results:

  • Statistically significant improvements were observed in both FM and WMFT scores from pretest to posttest (Wilcoxon signed-rank test).
  • Patient and therapist surveys indicated MP is appropriate and feasible for use in AIR.
  • Acceptability scores for MP were lower compared to appropriateness and feasibility.

Conclusions:

  • Mental practice appears to be a feasible and appropriate intervention for addressing severe upper extremity hemiparesis in the acute inpatient rehabilitation setting.
  • Further research may explore strategies to enhance the acceptability of MP among patients and therapists.
  • MP offers a potential avenue for enhancing motor recovery in early stroke rehabilitation.
