Background:

  • Giant cell arteritis (GCA) is a leading cause of preventable vision loss in the elderly.
  • Acute, painless vision loss necessitates prompt evaluation for GCA.

Purpose of the Study:

  • To evaluate the diagnostic utility of bilateral temporal artery biopsies (TABs) in suspected cases of GCA presenting with vision loss.
  • To discuss the diagnostic challenges and management strategies for GCA.

Main Methods:

  • Case presentation of an 89-year-old male with acute vision loss.
  • Clinical evaluation including neurological work-up and inflammatory markers.
  • Bilateral temporal artery biopsies (TABs) performed after initiating corticosteroid therapy.

Main Results:

  • Left eye vision was light perception with optic disc edema.
  • Histopathology of the left TAB was negative for arteritis.
  • Histopathology of the contralateral (right) TAB was positive for arteritis.

Conclusions:
