Background:

  • Rheumatoid arthritis (RA) is a systemic autoimmune disease that can affect bone mineral density (BMD).
  • Mandibular bone assessment using indices like MCI and WI offers a potential non-invasive method to evaluate systemic BMD changes.

Purpose of the Study:

  • To compare mandibular bone mineral density (BMD) in rheumatoid arthritis (RA) patients versus healthy controls using specific radiographic indices.
  • To evaluate the risk of low BMD in RA patients by assessing the mandibular cortical index (MCI), W-index (WI), and mandibular canal cortices continuity.

Main Methods:

  • A retrospective comparative study included 37 RA patients and 37 matched healthy controls.
  • Mandibular cortical index (MCI), W-index (WI), and mandibular canal cortices continuity were assessed by two experienced observers.
  • Conditional logistic regression analysis was used to identify associations between MCI and other variables.

Main Results:

  • While most RA patients exhibited altered MCI, no significant differences in W-index (WI) related to BMD were detected between groups (p = 0.54).
  • Odds ratio analysis revealed no association between lower MCI and the presence of RA, ethnicity, sex, glucocorticoid use, bisphosphonate intake, or mandibular cortices continuity.

Conclusions:

  • Most rheumatoid arthritis patients displayed altered mandibular cortical index (MCI), suggesting potential bone changes.
  • Despite altered MCI in RA patients, the study did not find significant associations between lower MCI and RA, or other investigated clinical and radiographic variables.
