    Summary

    High-definition transcranial direct current stimulation (HD-tDCS) can improve upper extremity function after stroke. Simultaneous anodal-cathodal HD-tDCS showed benefits, but not superior to separate stimulation methods.

    Background:

    • Stroke frequently impairs upper extremity function, impacting quality of life.
    • High-definition transcranial direct current stimulation (HD-tDCS) shows promise for motor recovery.
    • Previous studies investigated anodal or cathodal HD-tDCS effects separately.

    Purpose of the Study:

    • To investigate the effects of simultaneous anodal-cathodal HD-tDCS on upper extremity function post-stroke.
    • To compare the efficacy of bilateral HD-tDCS with unilateral anodal and cathodal stimulation.
    • To explore the impact on motor-evoked potentials (MEPs) and clinical scores.

    Main Methods:

    • Five stroke survivors participated in a within-subjects design.
    • Four conditions were tested: anodal HD-tDCS, cathodal HD-tDCS, bilateral anodal-cathodal HD-tDCS, and sham stimulation.
    • A two-week washout period was implemented between the four experimental visits.

    Main Results:

    • All active stimulation conditions (anodal, cathodal, bilateral) significantly improved Fugl-Meyer upper extremity scores.
    • Active HD-tDCS reduced the latency of ipsilesional M1-induced MEPs.
    • Bilateral stimulation did not demonstrate a significantly greater effect compared to unilateral stimulation.

    Conclusions:

    • HD-tDCS is a viable method for enhancing upper extremity motor function post-stroke.
    • Simultaneous bilateral stimulation may not offer additional benefits over separate anodal or cathodal application.
    • This research contributes to understanding neural plasticity and optimizing HD-tDCS protocols for stroke rehabilitation.
