    Summary

    Severe delirium tremens causes a hyperdynamic cardiorespiratory state with increased cardiac output and oxygen consumption. This compensatory response may require additional therapeutic support.

    Background:

    • Delirium tremens (DT) is a severe manifestation of alcohol withdrawal.
    • Hemodynamic and oxygen metabolism patterns in DT are not well-defined.

    Purpose of the Study:

    • To characterize the hemodynamic and oxygen metabolism patterns during severe delirium tremens.

    Main Methods:

    • Examined cardiorespiratory variables in five patients.
    • Compared data during DT, a control period, and postresolution.

    Main Results:

    • Significant increases in cardiac index, left cardiac work index, oxygen delivery, and oxygen consumption during DT compared to control.
    • No significant differences between control and postresolution periods.

    Conclusions:

    • Severe delirium tremens is associated with a hyperdynamic cardiorespiratory state.
    • Increased cardiac output during DT may compensate for elevated oxidative metabolism.
    • This state may necessitate additional therapeutic interventions.
