    Background:

    • A 21-year-old male presented with petrous apicitis, extradural and retromandibular abscesses, and facial nerve paresis.
    • Initial surgical interventions and broad-spectrum antibiotics were ineffective.
    • The patient developed labyrinthitis, sensorineural hearing loss, and meningitis.

    Findings:

    • Intraoperative biopsies revealed tuberculoid granulomas, confirming tuberculosis.
    • The infection responded effectively to triple-drug anti-tuberculosis therapy.

    Implications:

    • Tuberculosis of the temporal bone can be an aggressive infection, not always indolent.
    • This case highlights the importance of considering tuberculosis in complex skull base infections.
    • Effective management involves prompt diagnosis and appropriate multi-drug therapy.
