We report the case of the fortuitous discovery of a posterior mediastinal mass with a fatty component, not immediately identifiable on the PET scan. Because of the atypical appearance on imaging and the lack of diagnostic certainty, the patient received surgical management, which has ultimately made it possible to accurately diagnose and curatively remove extra-adrenal myelolipoma.

Most often incidentally detected, extra-adrenal myelolipoma is a rare tumor with a good prognosis. Management is primarily surgical. Monitoring of the lesion depends on its size and location.
