Identify career support systems for full-time occupational physicians in large Japanese companies.
Examine how companies foster long-term contributions from occupational health staff.
Understand the current landscape of career development in corporate occupational health.

Main Methods:

Conducted interviews with supervising occupational physicians.
Utilized snowball sampling for company selection.
Included companies with over 10,000 workers and more than 10 full-time occupational physicians.

Main Results:

Eight companies (six manufacturing, two railroad) participated.
All companies had systems for expertise development, increased responsibility, and career progression.
Company efforts focused on human resource development, expertise maintenance, and career stage-based treatment.

Conclusions:

Effective career support is essential for occupational physicians.
Enabling long-term contributions requires structured career development pathways.
Stable, high-quality occupational health services depend on physician career growth.
